EasyDocForms is built for independent practices that need compliant forms, signatures, assessments, notes, and scheduling in one workflow. It helps practices replace paper packets and disconnected handoffs with structured intake, consent, charting, and visit documentation that follows the patient through the process.

The platform centers on uploading existing PDF forms and turning them into mobile-friendly workflows without using a complicated form builder. Practices can create consent forms, complex intake packets, body diagrams, ABN forms, and outcome assessments, then use conditional logic based on service, visit type, or patient answers. E-signatures are built in, and patients can sign on their phones.

For documentation, the product supports simple chart note templates, an iOS app for charting from an iPhone or iPad, supervisory chart review for NP and PA notes, medication barcode scanning for lot numbers and expiration dates, and AI speech recognition. Scheduling can be handled with the built-in scheduler or synced with Square Appointments, and forms can be mapped to specific services or visit types so patients receive the right paperwork automatically.
